Dear State of Hawaii DOE Employee: To submit a trouble ticket, carefully fill out the form below and click the "Submit Trouble Ticket" button.  You will then see a "Submission Received" confirmation on the screen.  If your confirmation does not appear immediately, be patient and do not keep clicking the "Submit Request" button.
